How do I complete a Letter of Agency for Helix Telecom?
Helix Telecom requires a Letter of Agency completed and signed when your port request may be rejected due to inadequate documentation. If you are instructed to return and sign a Letter of Agency please follow the instructions below.

Federal Law requires the following fields on the LOA to be hand written and completed by the account owner:
- First Name
- Last Name
- Address
- City
- State
- ZIP
- Phone Number(s) being ported
- Signature
- Printed name
- Date LOA is completed
The Business Name field must be completed if the number belongs to a commercial account with your current carrier.
The Service Provider column must contain “Helix Telecom” for each number being ported to Helix Telecom.
